AFTO Forms/Labels

Does anyone know where I can get a clear copy of the following AFTO forms/labels.  Originals would be better but a clear copy would be good.

AFTO Form 108, TMDE Certification Label
AFTO Form 394, TMDE Certification Label (Small version)
AFTO Form 99, Limited/Special TMDE Label
AFTO From 398, Limited TMDE label (Small version)
AFTO Form 249, TMDE Calibration Data form
AFTO Form 250, Calibration Correction Chart
AFTO Form 255, Notice Calibration Void When Seal is Broken Label.

I've found them online in TO 00-20-14 but whoever scanned them into this document didn't do a very good job.  When I copy them to a word file and expand them they are not very clear.
